Delivered Directly in Your Home

WE COME TO YOU

Care happens where life happens. In-home therapy allows us to assess and address real-world challenges in your actual environment — your kitchen, bathroom, bedroom, and daily routines.

Function-First, Whole-Person Approach

COMPLETE SUPPORT

We support physical, cognitive, emotional, and environmental well-being — not just isolated symptoms. Occupational therapy allows us to intervene across habits, routines, cognition, environment, and culture.

Individualized Treatment Plans

UNIQUE EXPERIENCES

Every plan is tailored to your unique needs, goals, and daily life. We use evidence-based frameworks including Skills²Care® and COPE to build sustainable strategies for lasting progress.

Caregiver Education & Training

HANDS-ON LEARNING

We work alongside care partners with hands-on education, coaching, and resources that empower you to support your loved one confidently between sessions.

Clinical Coordination

COLLABORATIVE CARE

We collaborate with neurologists, primary care providers, memory clinics, and interdisciplinary care teams to ensure continuity of care and translate medical recommendations into daily function.

We Support A Range of Conditions

and circumstances

Specialized care across a wide range of neurological, orthopedic, and age-related conditions

Neurological Conditions

Stroke, traumatic brain injury, spinal cord injury, multiple sclerosis, and cerebral palsy. Interventions focus on restoring functional independence, promoting neuroplasticity, adapting tasks/environments, and supporting both the individual and caregiver.

Alzheimer's, Dementia & Parkinson's

All types of dementia, Alzheimer's disease, and Parkinson's/related movement disorders. Services emphasize functional cognition, environmental modification for safety, behavioral strategies, caregiver training, fall prevention, and meaningful engagement. This is the core specialty — using evidence-informed approaches like Skills²Care® and COPE.

Orthopedic Conditions

Joint replacements, fractures, rotator cuff injuries, arthritis/degenerative joint disease, and chronic pain. Focus on restoring functional movement, joint protection, energy conservation, and adaptive equipment training.

Post-Surgical Recovery

Limb amputation, spinal surgery, spinal cord injury, complex orthopedic and neurological surgeries. Includes ADL retraining, functional mobility training, home safety assessment, and transition planning from hospital to home.

Balance & Fall Prevention

Functional balance assessment, home safety evaluation, environmental modification, strength/coordination strategies, assistive device training, and education on fall risk reduction. Addresses both physical and cognitive contributors to fall risk.

Cognitive Changes

Mild cognitive impairment, post-stroke cognitive deficits, brain injury-related changes, early-stage dementia, and functional cognitive decline. Includes structured routine development, compensatory strategies, and safety planning.

Vision & Sensory Changes

Visual field deficits, depth perception challenges, contrast sensitivity changes, sensory integration concerns, and peripheral neuropathy. Services include environmental adaptation and visual compensatory techniques.

Age-Related Decline

Reduced endurance/mobility, increased fall risk, difficulty managing complex daily tasks, emerging cognitive concerns, and transition planning for aging in place. The proactive approach focuses on preserving independence and preventing avoidable decline.
